If you were not at fault for the accident
Notice of Accident Claim Form (Non-Fatal)
Complete this form if you have personally suffered an injury due to the fault of
the owner or driver of a motor vehicle.
Notice of Accident Claim Form (Fatal)
Complete this form if you are a relative/dependant claiming for loss resulting from
a person sustaining a fatal injury due to the fault of an owner or driver (other
than the deceased person) of a motor vehicle.
